  For access to physical resources, complete the Library Application Form. For access to digital resources, complete the eJournal Access Application Form. Please be aware that all application forms are processed manually during business hours only, and that delays outside of our control may occur.

• Student Card Form
  Please fill out the Student Card form to receive your official AC student card.
  
• Refund Request Form
  Please complete the Refund Request Form if you believe you are entitled to a Refund.
• Letter of Release Request Form
• Application for NSW Student Travel Concessions
  Please complete the Application for NSW Student Travel Concessions if you believe you are entitled to a Student Travel Concession. Complete this form and submit it to the Registrar’s Office either:
  • In person at AC reception (AC Central, 30 Cowper Street, Parramatta NSW); or
  • As a scanned email attachment to  VETregistry@ac.edu.au
